Health Information Privacy Complaint

According to the Health Information Privacy Complaint Form OMB No. 0990-0269, Complaint Consent Form, The Department of Health and Human Services’ (HHS) Office for Civil Rights (OCR) has the authority to collect and receive material and information about a complainant, including personnel and medical records may be relevant to an investigation of a person’s complaint. In order to investigate the complaint, OCR may need to reveal the person’s identity or identifying information about a person to persons at the entity or agency under investigation or to other persons, agencies, or entities. Consent is voluntary and it is not always needed in order to investigate a complaint; however, failure to give consent will likely impede an investigation of a complaint and may result in the closure of the case (HHS.gov web, n.d.).
